The Clinical Assessment Nurse-RN is responsible for conducting the clinical evaluation of residents to assure individual resident medical, psychosocial, emotional, and physical needs are identified, and plans are developed to assist residents in achieving their highest practicable level of function

Responsibilities and Duties
- Conducts clinical evaluation of the residents condition.
- Utilizes the assessment reference date assigned by the MDS Coordinator/DON.
- Attends management level meetings to ensure standards of practice and continuum of care needs.
- Conducts bedside interview and assessment of assigned residents prior to completing the MDS and CAAs to assure accurate assessments are completed and plans of care reflect the individual needs and preferences of the residents.
- Monitors point of care for assigned residents and assists, educates and supports staff to accurately document residents care.
- Communicates with the resident, representative and or family the outcomes of assessments and plans of care to assure opportunities for input and adjustments can be made.
- Assists to coordinate and integrate resources needed to accomplish the plan of care.
- Evaluates the effectiveness of the plan of care and documents as needed or scheduled progress/barriers in researching desired outcomes.
- Encourages collaboration among interdisciplinary team members and the resident to ensure a holistic and coordinated approach to resident care and services is achieved.
- Completes the level of care determination accurately and timely for eligible residents.
- In the absence of the MDS Coordinator, obtain authorizations or re-authorizations for assigned residents.
- Attends and participates in committee meetings assigned by MDS Coordinator.
Qualifications and Skills
- Registered Nurse with a minimum one-year nursing experience in a skilled nursing/long-term care facility.
- Excellent verbal and written communication skills with residents, families, all levels of staff and external customers.
- Proficient computer skills are essential including the ability to use Point Click Care and conduct online submissions.
- Must possess the ability to work harmoniously with and supervise other personnel.
- Must possess a high degree of personal accountability, responsibility and the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc. that are necessary for providing comprehensive activities to residents.
