MDS Coordinator Team Lead
MDS Coordinators are healthcare professionals that oversee providing the best options for residents in terms of care delivery in facilities that are accredited or offer Medicaid and Medicare services and evaluate the quality of emotional, mental, and physical care being given to long-term care residents. Under this process, the MDS Coordinator is tasked to ensure that the resident services are well documented during the reference period. The resident assessment details will also be used to formulate the best care plan for the residents highest practicable wellbeing. The MDSC is also responsible for ensuring an accurate record per RAI manual resulting in optimization of State CMI for reimbursement purposes. The MDSC Team Lead takes responsibility for directing and managing MDSCs to ensure effective efficient utilization of the RAI process.
- Coordinate resident care with MDSCs and oversee the assessment process, setting the assessment schedule and assures that assessments are completed in an accurate and timely manner.
- Analyzes resident records and condition, and in conjunction with IDT determines legitimacy of admission, treatment, and length of stay which accurately comply with government and insurance reimbursement policies.
- Participates in quality assurance practices.
- Performs coding of MDS which ensures accurate and complete depiction of resident condition and reimbursement modalities.
- Review MDS validation reports for accuracy and approve or correct and resubmit according to Resident Assessment Instrument (RAI) guidelines.
- An MDS Coordinator Team Lead coordinates the care plan with the interdisciplinary team (IDT) in accordance with regulatory requirements. They participate in identifying resources that need to be made available in conjunction with the resident's overall plan of care.
- Work in collaboration with the DON/Administrator/IDT to ensure accurate and timely completion of functional abilities, diagnosis sequencing, and information contained in referenced by the MDS.
- MDS Coordinator Team Lead have a working knowledge of Medicare replacement and managed care plans and provide oversight of the preauthorization process, when necessary. Provide oversight of appropriate consultation and referral to case management personnel.
- Maintain accurate records of all communications and interventions.

Qualifications and Skills:
- Board certified licensed Registered Nurse (RN) prefer RAC-CT.
- Clinical experience in healthcare / long term care setting.
- Demonstrated knowledge of state and federal regulations as it pertains to clinical care in Long-term setting.
- Ability to accurately complete timely and accurately the MDS item set.
- Ability to apply common sense understanding to carry out instructions furnished in written or verbal form.
- Ability to diffuse complaints in a professional and timely manner with family, clinical professionals and third parties and further assist with promoting quality of care and/or life.
- Promote cost effective medical outcomes.
- Promote decreased lengths of hospital stays when appropriate.
- Prevent hospitalization when possible and appropriate.
- Assure appropriate levels of care are received by residents.
- Ability to follow instructions, read, and interpret documents for operating, training, and disseminating to others.
- Proficient working knowledge of PCC and Microsoft Office Products.
Working Conditions and Physical Demands:
- This is a full-time position and overtime may be required.
- Travel will be minimal.
- Ability to lift seventy-five (75) pounds.
- Moving, lifting, or transferring patients may involve lifting up to 100 pounds.
- Ability to stand for extended periods of time.
- Fine motor skills.
- Visual acuity
